Principles of Pain Management: 1.
In order to alleviate pain, its immediate causes have
to be identified precisely, and treatment concentrated
on eradication of the etiological factors. These usually
consist of tender spots, trigger points, muscle spasms
and/or inflammation.
2. Long-term relief of pain can be
achieved only by eradication of its causes. In case
of tender spots, trigger points, and taut bands associated
with them in muscles, a “fibrotic core”
exists which can be cured only by mechanical breaking
up using the Needling & Infiltration technique.
3. Since Spinal Segmental Sensitization
(SSS) is consistently associated with pain regardless
of its etiology, one basic goal of treatment should
be reversal of the segmental sensitization. Paraspinous
block achieves this goal promptly to normal by blocking
the nociceptive impulses mainly arising from the irritated
interspinous ligaments.
4. Long-term relief can be assured only
by eliminating the etiological and perpetuating factors
that caused the spasm and TSs/TrPs. The injections including
the Paraspinous Block to desensitize the segment as
well as Needling & Infiltration in order to break
up the peripheral taut bands, and trigger points open
the possibility for rehabilitation, and treatment. |
